It appears that the icons didn't get installed with the module.

The only image I see in the resources.zip file is icon_UserImport_32px.gif

These are actually images that should appear in your dotnetnuke\images folder... Are they there, do they not show up in the module?

The problem is with a different module, not Interactive User Import.

Once again, "never mind."
