Regardless of the state of the "Add field to same row as previous question?" checkbox or the settings on having the field label above the field or beside or the skin on my site or the location of the module on the page, I have a single column of field labels and fields.

I think there is some confusion here because of the new features related to tables and DIV's (this feature requires the tables feature to be enabled). Can you please go to control panel, module configuration, and in the "Form Layout" section, can you change this feature to be set to "Tables" instead of being said to "DIV's"?
