I need to create a survey in which the user rates services from 1 to 5. I'd like to include a "Rating Score" that is the average of the questions that were rated.

I'm not sure what field type to use for the rating questions nor how to calculate the average.

What you are trying to achieve sounds very similar to a Demonstration we have available. It's not doing exactly what your describing, but you can use the logic from this demo to construct what you're trying to do.
