I am currently using Dynamic Forms 3.40.41 DNN 5.06.01

When I was working with my dynamic forms I found that there is  a problem with my dynamic email completion event. When the form goes to send off the email for some reason the Email and the name get combined and I get this error message in the event log.

For some reason it will not send the form off like this. Does anyone have any ideas?
