Is there a way of layout out the fields in the form? Right now it defaults to top down, but I would like to arrange the fields so that it makes sense. Is there a way to do this? Thanks.

You can go into the control panel and look at 'manage questions' then when you are there you will find a 'question look/feel' area. When you click that box you will find a setting that says: [L]Add field to same row as previous question?. This setting will allow you to place the next question in the same row as the previous question. Try that out and see if working with it gives you what you are trying to achieve.
