    I'm new to Dynamic Forms and just installed it.  On my very first attempt to produce a form, I get this message:

    Error using these files...Index was outside the bounds of the array.

    Anything I should be worried about?  Is something just not hooked up because I'm running on a 20 day demo?

    Terry - Can you please provide a few more details on your implementation? Specifically:

    1. What version of DNN are you running on? Any specific hosting platforms (i.e. AZURE or something that is not SQL Server?).

    2. What specific error do you see under Admin, Event Viewer?

    On a test machine with really nothing else on it
    I'm running Windows7, IIS 7.5

    I'm running DNN Community 6.00.02 (72)

    Data Provider:  SQLDataProvider

    .net:   3.5


    Dynamic Forms:   4.0.11.16442

    Again here is the error message:
    "Error using these files...Index was outside the bounds of the array."

    Since I posted this earlier this afternoon, I've had to put the Dynamic Forms Module down for awhile because everytime I click on the icon to edit a field, and it takes me to the Dynamic Forms Control Panel, I get a popup message saying "Script is causing IE to run very slow, do I want to stop the script or keep going".  I've learned already to just say "stop" otherwise my computer develops issues, and does run very slow and ultimately becomes unresponsive.

    Oh, and I looked at the logs in the admin folder and didn't see anything out of the ordinary.  Just things like the IP from where I would log on, etc.

    Hi Terry - I am not sure what is going on as this is the only report of this at this time... You might go to Admin, Event Viewer and see if you see any unusual errors there.

    Hi - I too am having this issue. I am running DNN 6.1.3 and just upgraded to DF 4.0. 

    After upgrading, I receive the error message of "Error using these files...Index was outside the bounds of the array." it happens every time I try to fill out a specific form on our site that auto fills in one field based on the value chosen for a previous field. As soon as it goes out to check the database to run the sql script, the page refreshes with four lines of this error message on it. 

    This is a form we have been using for a couple of years with no previous issues until now.

    Also wanted to note that when I run in debug mode I receive the following:

    SQL Debug (Parse SQL Text For Variables):select pin as questionoption, pin as questionoptionvalue from hwep_DynamicForms_VFC_Providers where Pin like'1%'

    So apparently there is something wrong with my SQL scripts, but they were working just fine before the upgrade to DF 4.0. PLEASE HELP!!

    Hi Julie,

    Trying to narrow it down with the other reports. Do you have a file upload field?

    Also, would you please download the latest build of Dynamic Forms 4.0.12? You'll find this in the DNN Store under Patches if you bought the module individually or in MyDownloads for the Collection 5.0.

    Candace - my apologies, we do have the 4.0.12 version installed already. And, yes - these particular forms do also have a file upload field. I was able to successfully upload a very small PDF, but others who have tried were not able to upload a file (this also resulted in the YSOD). But, even when I was able to upload a file, I still get the "Error using these files...Index was outside the bounds of the array" error message - it actually appears right after entering the data for one field that then checks the database and runs the SQL query to populate the next field based on the value entered for the first field.

    Hi guys - We have this fixed, if you open a support ticket I can get you the updated patch (please reference this thread), otherwise we are planning a full release by the end of March.

    I've submitted two tickets to get the upgrade - I assume 4.1 is what you (Administrator) were talking about and it fixes this issue. But I'm not getting an email acknowledgement that the tickets were even accepted. I actually purchased dynamic forms after the last post on this thread, but am having this issue.
