How do I get the URL for the page the form is submitted from?

 

I tried creating a hidden field called TabID and checking the box to pass the query string.  However, the checkbox does not stay checked every time I update.

 

Is there an easier way to do this?

Actually I think the real problem is that any changes to a field after it is created are not saved.
