Ryan, This problem has taken a turn. I was looking at the page again, with the login that I gave you, and noticed one too many "Manage" buttons at the top left of the form. There should be one for the form and that is it. But there were two, stacked and off-set from each other. The one on top was for a feedback module that should have been below the form. Since it appeared to be misbehaving and covering up the form, I moved the feedback module to a lower pane. Presto. The misbehaving form was uncovered and is now visible. At this point I am guessing that the feedback module was doing a perfect job of covering up the Dynamic Form with the exact right dimensions to cover the form.
I still don't know why
1. the feedback form would cover up the dynamic form
2. How it could be so precise with it's dimensions to cover it perfectly
3.Why the feedback form would be clean and empty with no fields showing
4. Why it only did this in FireFox and not IE or Chrome or Safari.
5. Why this only happened to a few user accounts and not all of them.
I suppose I am not going to concern myself with finding these answers. I'm just happy to fix the form what wasn't actually broken. It appears there was nothing at all wrong with this form.
