New to Dynamic Forms - Doing a Survey Form
What would be best way create the type of question:
1) How often have you used each type of tobacco in the previous 60 days?
a. ____ cigarettes per day
b. ____ chewing tobacco per day
c. ____ cigars per day
d. ____ other tobacco products per day
On each line they need to be able to type an amount in. I don't see a default field that will allow me to set it up, so what steps do I need to do to create it. Thanks
