Well, you have a view options here...
1. Within the Dynamic Forms module configuration there is a setting called 'Summary Field' and within that area you can define what field you want to be the summary field. One thing you could do would be to define a hidden field that could be called 'MySummary' and use a client side event to set the value of your combination of fields. Something like:
$(MySummary) = $(FirstName) + ' ' + $(LastName) ' ' + $(ApplicationField) etc... You would want to put that client side event on any field that would affect the value such as in the firstname, lastname, application field client side events area under advanced field options. This way the hidden field would represent all of those fields combined and that would be your summary field.
2. Another option with much more power and flexibility would be to use Dynamic Views. Dynamic Views is another module you can find at
http://www.datasprings.com/Dynamic-Views and includes many more configuration options, ability to search, filter, and so forth... This module has a direct build in integration option with Dynamic Forms.
