Hello Brett, There are two tables "DynamicForms_Question" and "DynamicForms_QuestionResponse". The relationship is one to many. Here's a typical query for connecting the two tables together and showing the UniqueID of the Question, Question Name, and Value: select top 100 A.DynamicQuestionID, A.Question, B.Response from DynamicForms_Question A inner join DynamicFOrms_QuestionResponse B on A.DynamicQuestionID = B.DynamicQuestionID All Dynamic Forms on your site will save the form responses(form submission data) into the DynamicForms_QuestionResponse table, so you'll need to add a filter to your query to limit the results to the results to a specific Dynamic Form: select top 100 A.DynamicQuestionID, A.Question, B.Response from DynamicForms_Question A inner join DynamicFOrms_QuestionResponse B on A.DynamicQuestionID = B.DynamicQuestionID where A.ModuleID = 'TheModuleID' Please let us know if you have any questions.
