having trouble formating a form

    Hello I’m trying to figure out how to reproduce the below form in forms 3.1. But i can't seem to figure out how to get the form  to have a horizontal feel rather then everything just vertical I hoping someone has a sample template or can tell me how I can accomplish this task. I seem to be running into a limit of what I can put into the form when I get a couple pages deep. Then I wish it to email to a single address. if anyone could help me i would be really greatful.

    Hi Donovan,

    Let me point you to our Form Demos -- Demo 19 shows various layout options that you can do.  It may not look exactly as your example above but you can come close with alternatives.   Thanks!
