QUESTION 1:
You can accomplish this by going to your OWS Adminstrator, and open up your Query Template -> Detail Template. This template is expanded from Header Template.
From here double click on the Detail Template and you'll see some HTML and JavaScript. You'll notice an anchor tag around the text of "Edit". You will change the href to the form that you wish to pull in the data. You will do the full link to this page:

Now DBUpdate and DSParam1 will be hidden fields on your form that retrieve values from querystring.
Go to Module Configuration -> Initial SQL rendering/Bind and check the box "Enable Initial SQL Data Bind". Now in the field "Only enable when querystring value is present?" place DBUpdate. In "Initial SQL Query:" place SQL that fits your table but follow this example:
Select * from yourFormTableName Where UniqueCompletionID = $(DSParam1)
Note: An important part is to remember that all of your short field names of your questions must be the same as the columns in your table in order to use SQL binding.
QUESTION 2:
1.) Make a form with all of your fields.
2.) You will need to make a SQL form completion event and from here you will see links to generate table, generate insert statement, etc...
Click generate table/create table.
3.) This will give you a create table SQL statement that goes along with all of your fields on your form, columns are named - by the short name of your fields.
4.) Change the table name you're creating to something that has meaning to what you're doing.
5.) Place this statement in HOST-> SQL and execute.
6.) Now you have a table that you created, so when ever you have SQL completion events make sure they're quering this table you created.
ALSO:
Make sure you make a SQL completion event that will only fire if DBUpdate = True. That will make use of the edit/update of OWS to your database table that you created.
