I have managed to re-create this error a number of times with the following steps:
Version
Ver:3.30.61.0 / 5.5.0
Mid:1196 / Tab:397
1.Add new page
2.Add dynamic form
3.Add check box field Q1
4.Add text field Q2 - Hide until forced visible
5.Add text field Q3
6.Configure submit button to use custom image and to display to the right of the last field (e.g. Q3)
7.Add question event to show display Q2 when Q1 is checked.
View form and check Q1, fatal error occurs.... if you change the submit button config, all works okay again, problem with custom and show to right.
Also is you click hide submit button and show submit button to the right of last field, the submit button isn't hidden. (2 BUGS)
