Gridview / Survey Field last line not shown in Results table

    Hello All,

    I have been having some issues with a Gridview/Survey field in my dynamic forms form.

    The field has 5 options (see attatchment) and an option set for each for rating. The problem is the 5th one will not show. And if its set above any other question result, the other questions will not show.

    So if i put it below "Name" and "Gender" Text fields, but above the "Age" and "Address" Text fields. I will see "Name" and "Gender" fields. But i will not see "Age" or "Address" fields.

    Has anyone had similar issues with this field type?
    If so, are there any tricks with this i may be missing?

    If anyone can help with this i would be very grateful.
