    Hello Kaustav,

    I would recommend using a Dynamic Form. First start by creating all of the Questions needed.

    Then you can begin adding in Calculations using JavaScript. Regarding JavaScript Calculations and Dynamic Forms, I would recommend taking a look at Demonstration 20 on our site:

    Implementing JavaScript can be cumbersome if you don't understand JavaScript. JavaScript is a great programming language for client side events. When implementing JavaScript in Dynamic Forms, it's important to also understand the debugging techniques in case you have erroneous code.
